SimCapture Enterprise with Exam System: Remote OSCE basics
Discover the fundamentals of implementing a Remote OSCE with SimCapture Enterprise and Exam System.
Table of Contents
This article will cover the three main workflows of a Remote OSCE interaction in SimCapture Cloud: The Proctor Workflow, the Learner Workflow, and the Standard Patient Workflow. We will discuss what should be seen by each user and the steps to complete a Remote OSCE successfully.
Firefox is not supported for Remote OSCE. Please use the Chrome web browser to gain access to exams.
Hardware requirements
- A desktop/laptop computer with an up-to-date web browser (this is not Mac or Windows Specific)
- Microphone (computer mic or third-party mic will work)
- Camera (computer camera or third-party camera will work)
Software requirements
- SimCapture Cloud-Supported Web Browser. Laerdal recommends Google Chrome.
Network and bandwidth requirements
- 1.2Mbps (up/down) minimum required
- Network issues may be seen on restrictive or guest networks pending firewall settings
The proctor workflow
Cloud users with the Control Interface access will see the Remote OSCE symbol when this is enabled for the client.
Once selected, the Proctor will see an interface very similar to the standard Control Interface. This includes:
- Ability to bulk action rooms such as starting the encounter and opening the room for Learners.
- Select individual rooms via a list.
- Select individual rooms and configure via box visuals.
- Ability to add rooms on demand.
As a Proctor, configuring each room will have a similar dialogue box to CCM's. This will allow the Proctor to set the Course, Scenario, Patient, Learner, and display color in CCM.
Note: When running multiple Remote OSCEs in the same room, SimCapture will preserve the Course, Scenario, and Patient information unless the Proctor clears this configuration.
As both a Learner and Standard Patient enter the Remote OSCE workflow, the Proctor may see the following symbols in each room:
-
The participant is not connected to the exam interface. This could be an audio and/or camera issue if this is unexpected.
-
The participant is present and in the virtual room.
Note: A Proctor can use the Monitor Interface during an exam. At publishing, only view permission is available within remote rooms.
The user monitoring a virtual room must have the Monitor Permission at the Course Level. This can be found within Course > People > Monitors.
The learner workflow
Under the My Courses Pod, a Learner will now see the Remote Exam: Learner button.
If this is selected by the Learner when they are not configured to a room, they will see a message stating, "You have no exams to take at this time."
However, the Learner will select the camera microphone and see a preview of each to prep for an upcoming exam.
Once the Remote OSCE Proctor has placed the appropriate Learner into the room, the Learner will then see the message of "Please wait, Your remote exam in the room"_____________" is about to start." Again, the Learner will be able to select their Camera and Microphone and see a preview of each before starting the exam.
Once the Proctor has opened the room for Learners, the Learner will see the Begin Exam button at the top of the page. Once pressed, the Learner will move to the first stage of the exam flow, such as the Honor Code, Door Note, or EMR.
Once the Learner has advanced and the Proctor has begun the Encounter portion of the exam, the Learner will see a screen similar to the one below. This will include:
- The Virtual Room
- The First and Last Name of the Learner
- The remaining time in the encounter
- The Standard Patient Video
- The Learner Video and audio
- The EMR, if applicable
- The Stop Encounter Button
When provided an EMR, Learners will have this brought to the forefront to complete while still viewing Learner, and Standard Patient video feeds.
Once the encounter and any follow-up evaluations and/or recordings have been completed, the Learner will be brought back to the Remote Encounter Waiting Room and exit the screen without issue to save progress.
The standardized patient workflow
Similar to the Learner, within the main page of SimCapture, Patients will see the remote Exam: Patients button at the bottom of My Courses.
Once selected, the Patient will enter a virtual waiting room to select their camera microphone and ensure all are working appropriately in the preview to the left.
If they are configured for a virtual room by the Proctor, the Patient will enter the room at any time. While waiting for the Learner, the Patient may see messages such as Waiting for Student to log in or Waiting for Student to enter. During this time, the Learner progresses through the Honor Code, Door Note, and/or waits for the Proctor to open the room for the encounter to begin.
Once the Proctor has begun the encounter, the Patient will see a similar screen to the Learner. This includes
- The Virtual Room name
- The role of the user
- The first and last name of the user
- The time left in the encounter
- The Learner Video feed
- The Patient Video and audio preview
Note: The ability to stop the encounter is not within the Patient's screen and is only for the Learner.
Once the encounter has been completed and any follow-up evaluations and/or encounters have progressed, the Patient will navigate back to the Virtual Waiting Room and be able to re-enter the room in the case of additional encounters.
